Thrive Community Options (TCO) provides Supported Living Services (SLS) to individuals receiving support through the HCBS-SLS waiver. SLS is intended to provide home and community-based support to persons living independently or in their family home. Services are designed to assist persons to maintain their health and safety, and maintain/build the skills necessary to live as independently as possible. TCO offers the following SLS support to persons and their families:
Personal Care (PC)
TCO provides PC services designed to support persons who require assistance with activities of daily living. PC services are provided in the person’s home and allow the person to maintain their health and well-being. PC services may involve hands-on support to complete the personal care task for the person or providing cueing to prompt the person to complete a personal care task. The level of support required will be identified in the person’s Individual Plan (IP). The following support may be provided as part of PC services:
- Ambulation/Transferring Support
- Bathing/Washing
- Dressing
- Eating/Feeding
- Grooming
- Bowel/Bladder Care
- Personal Hygiene
- Skin Care
- Medication Assistance
- Medical Appointment/Scheduling Assistance
- Money Management
- Menu Planning
- Grocery Shopping
Homemaker – Basic (HM-B)
TCO provides HM-B services designed to support persons who require assistance to manage basic household tasks. HM-B services are provided in the person’s home and allow the person to maintain a healthy, safe, and clean home environment. HM-B services may involve completing the home task for the person or providing cueing to prompt the person to complete a task. The level of support and tasks to be completed are identified in the person’s IP. The following household support may be provided as part of HM-B services:
- Laundry
- Cleaning/Changing Linens
- Folding/Ironing/Organizing Clothes
- Dusting Furniture/Shelves/Surfaces
- Vacuuming Carpets/Throws/Rugs
- Washing/Mopping/Cleaning Floors
- General Bathroom Cleaning
- General Kitchen Cleaning
- Cleaning Windows (Indoors)
- Trash Removal
- Sorting and Organizing
- General maintenance
Homemaker – Enhanced (HM-E)
TCO provides HM-E services that allows the person to receive support with household tasks necessary for the person to remain healthy and safe in their home. Enhanced Homemaker Services consist of the same household tasks as described under HM-B with the addition of either habilitation or extraordinary cleaning provided by a qualified homemaker. HM-B may involve extraordinary/deep cleaning or habilitation designed to support the person to develop the skills necessary to maintain their living environment. Habilitation involves developing step-by-step instructions and learning strategies designed to assist the person to develop the needed skills to maintain a safe, healthy, and clean home environment. The level of support and training needed or area(s) requiring deep cleaning are identified in the person’s IP.
Mentorship
TCO provides Mentorship services to individuals living in the community. Mentorship services are designed to support individuals to develop self-advocacy skills. Service delivery may include providing exposure to community environments and experiences, interviewing potential providers or generic support professionals, and assistance with participation on private/public boards, advisory groups or commissions. TCO provides Mentorship services in the community at the time and location chosen by the individual. Individuals design and determine their goals and objectives with support from TCO.
Respite
TCO provides Respite services to individuals living in the community. Respite is designed to allow individuals the opportunity to remain in their own home while their family member/caregiver is away. Respite services support the person to feel safe, nurtured and respected, in their home, at the same level of care as their family member or regular caregiver provides. Respite services allow family members/caregivers the opportunity for needed time away and allow individuals the opportunity to remain in their home and access their community safely.
