Thrive Community Options (TCO) provides Individual Residential Services and Supports (IRSS) to individuals wanting to live as independently as possible in the community of their choice. IRSS uses a variety of living arrangements individually designed to meet each person’s needs. The individual may live in a home owned or leased by the agency, their own apartment/home, their family home, or a Host Home. TCO assists individuals 18 years of age and older to find a place to live as well as training in areas which promote independence, self-sufficiency, and allows the person to participate successfully in their community. Individuals receive care and support with their activities of daily living, including: bathing, dressing, toileting, mobility, meal planning, medication administration, shopping, going to medical appointments, and other activities.

TCO offers a variety of living environments – depending on individual need and preference – for the person to choose. The type of setting individuals live in is determined by their personal preference, level of support needed, and supervision required. TCO offers Residential services through four models:

Host Home Model

Host homes allow individuals the opportunity to live in a private family home, with a qualified caregiver and their family.  Host Home Providers are independent contractors opening their homes to provide services for up to two (2) individuals. A third (3) individual may live in the home with a Third-Bed Waiver.  TCO coordinates services by recruiting and training qualified providers, performing background checks, managing ongoing program coordination, and monitoring the home environment.  Host Home Providers (HHP) assist individuals with developing their skills and building relationships in the community. 

Apartment-Living Model

Individuals who want to live more independently and do not require 24/7 on-site support have the option of living in an apartment. Our apartment program equips individuals with the skills to maintain their independence and build self-sufficiency. A level of weekly staff supervision is provided based on the unique needs of each person. TCO provides 24/7 access to staff and on-going support to live and thrive!  

Personal Care Alternative (PCA)

PCA’s allow individuals to live in a small (1-3 person) residential home setting with rotating staff support. Individuals gain the opportunity to live with other peers, under staff support and guidance. PCA’s can have a maximum of 3 persons with IDD living in a home. TCO controls (rents/owns) the living environment and provides rotating 24/7 staff support and supervision.

Family Caregiver Model (FCG)

FCG allows a person to remain living with a family member and receive Residential services in their family home. The person’s family member is paid as an employee (or contractor) to provide 24/7 care and support. TCO trains the family member, monitors the home environment, provides ongoing program management and service coordination.
