Transportation to Day Program, social outings and back to client’s home is provided by Thrive.

Regular safety checks and maintenance of our 8 vehicles is conducted on a regular basis.

Living Environments

A variety of living environments – depending on individual need and preference – are provided. The type of setting individuals reside in, is determined by their personal preference, level of support needed, and supervision required. Through employee assistance, Independent Contractors (Host Home Providers), or Family Caregivers, clients will be taken care of on a daily basis.

These opportunities / arrangements are made possible through Residential Habilitation Services and Supports (RHSS) and Residential Habilitation-Individual Residential Services and Supports (IRSS).

Thrive Community Options LLC., will provide Individual Residential Services and Support through:

Host Homes

Provide individuals with an opportunity to live in a private family home, with a qualified caregiver and their family. Host Home Providers are independent contractors opening their homes to provide services up to 3 individuals. A third individual may live in the home with a Third-Bed Waiver.

Thrive Community Options LLC., coordinates services by recruiting qualified providers, checks background and training requirements, provides ongoing program coordination and administrative monitoring.

Host Home Providers (HHP) assist our clients with developing their daily skills and building relationships in the community. Some HHP’s allow pets and client’s children to live in their residence with them.

Personal Care Alternative (PCA)

PCA’s allow clients to live in a residential home setting, but unlike host homes, they have a rotating staff. A little less family like, but some feel it’s a more independent setting.

Residents gain the opportunity to live with other peers, under staff support and guidance. PCA’s can have a maximum of 3 residents in a home. One to three individuals reside in a single-family home with a 24 hours on-site staff.

Apartment Setting

Individuals who require minimal assistance will have the option of living in an apartment. Our apartment program equips individuals with the skills to maintain their independence.

Level of weekly staff supervision is provided based on the unique needs of each resident. Thrive will provide 24-hour access to staff and on-going support to meet the independence-living skills of each resident.
